Filling out the Donor Trip Details Form gives us a better idea of who's going , when you would like to go and how to best make your travel arrangements. 

Be sure to print each travelers’ name exactly as shown on the title page of their passport (for international travel, including Canada) or driver license (if traveling wholly within the United States) on the enclosed original Award Confirmation. 


Please do not use any nicknames and avoid misspellings.


If a traveler is under age 18, not a US Citizen, or for any reason is not eligible for a US passport or US re-entry, please notify us. If there is any question as to who will be traveling, please wait until you are sure who will be on the trip to ensure we have accurate information.

So that we are all on the same page

1. Please complete the back of this original confirmation and mail to Fundraising Travel Foundation, Inc., Reservations Center, PO Box 540223, Orlando, FL 32854 by Trackable Mail to ensure delivery: Copies cannot be accepted. Please retain your tracking number as it is your protection against your certificate being lost in the mail. 

 

2. Since all trips and awards are registered to the award winner by name, we must receive this original copy to begin processing your award. If for any reason you have not received your trip information within 15 days of mailing this confirmation, please call us. Please ensure we receive this certificate at least 120 days before you desire to travel and use all awards within 3 years of event.

 

3. When we receive this confirmation with the passengers’ legal names (as on passport, or driver’s license if domestic travel), your personal Travel Concierge will contact within 15 days. You will have the opportunity to request travel date and advise us of special needs or dietary requirements for the airlines at that time. Please do not make other arrangements until your travel dates are confirmed.

 

4. All accommodations are at high quality, amenity-rich hotels and resorts with services you would expect at such a facility such as fully air conditioned private room or suite with private bath, 24 hour service, English-speaking staff, concierge service, daily maid service, full international telephone, internet and television service, etc. except where specified. You will have the opportunity to make rooming requests when making your reservations. 

 5. All air travel is on:

 a. Fully licensed major U.S carriers, (American, Delta, United, etc.) or

 b. International flag carriers, (British Airways, Lufthansa, etc.) or 

 c. Certain other fully licensed carriers, (Virgin Atlantic, etc.)

 

 We do not use charters or other non-traditional carriers. Although we do not know at this time which airline will be used (that depends on availability and convenience of service to your destination,) we will try to keep you on your airline of choice. 

 

6. Please ensure that your passports, visas, drivers’ licenses, credit cards and other documentation are up to date. If you need to acquire valid passports and/or visas for this trip, please begin the process as soon as possible. Passports must be valid for 6 months after your return date.

 

7. Airlines, hotels and U. S. Homeland Security do not allow passenger name changes. We strongly suggest you do not submit this form until you are absolutely sure who will be traveling. Please ensure the full legal names listed on the reverse side of this form are spelled clearly and correctly, and include dates of birth.
