Filling out the Donor Trip Details Form gives us a better idea of who's going , when you would like to go and how to best make your travel arrangements.
Be sure to print each travelers’ name exactly as shown on the title page of their passport (for international travel, including Canada) or driver license (if traveling wholly within the United States) on the enclosed original Award Confirmation.
Please do not use any nicknames and avoid misspellings.
If a traveler is under age 18, not a US Citizen, or for any reason is not eligible for a US passport or US re-entry, please notify us. If there is any question as to who will be traveling, please wait until you are sure who will be on the trip to ensure we have accurate information.
They will be your point of contact throughout the booking process and will walk you through every step of the way.
We will send you a safe and secure link to upload your travel documents and credit card authorization form.
Your trip will be sent over for approval. Once you approve all upgrades, taxes, fees and extensions we book your trip for you. We will also offer an optional Travel Protection plan. Then you are all set!
We love to hear feedback so any photos or reviews of our travel partners we would love to hear and see. If any emergencies should arise, we will provide you with a 24/7 phone number to help you through it.
So that we are all on the same page
1. Please complete the back of this original confirmation and mail to Fundraising Travel Foundation, Inc., Reservations Center, PO Box 540223, Orlando, FL 32854 by Trackable Mail to ensure delivery: Copies cannot be accepted. Please retain your tracking number as it is your protection against your certificate being lost in the mail.
2. Since all trips and awards are registered to the award winner by name, we must receive this original copy to begin processing your award. If for any reason you have not received your trip information within 15 days of mailing this confirmation, please call us. Please ensure we receive this certificate at least 120 days before you desire to travel and use all awards within 3 years of event.
3. When we receive this confirmation with the passengers’ legal names (as on passport, or driver’s license if domestic travel), your personal Travel Concierge will contact within 15 days. You will have the opportunity to request travel date and advise us of special needs or dietary requirements for the airlines at that time. Please do not make other arrangements until your travel dates are confirmed.
4. All accommodations are at high quality, amenity-rich hotels and resorts with services you would expect at such a facility such as fully air conditioned private room or suite with private bath, 24 hour service, English-speaking staff, concierge service, daily maid service, full international telephone, internet and television service, etc. except where specified. You will have the opportunity to make rooming requests when making your reservations.
5. All air travel is on:
a. Fully licensed major U.S carriers, (American, Delta, United, etc.) or
b. International flag carriers, (British Airways, Lufthansa, etc.) or
c. Certain other fully licensed carriers, (Virgin Atlantic, etc.)
We do not use charters or other non-traditional carriers. Although we do not know at this time which airline will be used (that depends on availability and convenience of service to your destination,) we will try to keep you on your airline of choice.
6. Please ensure that your passports, visas, drivers’ licenses, credit cards and other documentation are up to date. If you need to acquire valid passports and/or visas for this trip, please begin the process as soon as possible. Passports must be valid for 6 months after your return date.
7. Airlines, hotels and U. S. Homeland Security do not allow passenger name changes. We strongly suggest you do not submit this form until you are absolutely sure who will be traveling. Please ensure the full legal names listed on the reverse side of this form are spelled clearly and correctly, and include dates of birth.

Typically, flights will be on American, Delta, United or British Airways or one of their worldwide SkyTeam, OneWorld, StarAlliance, or their code share partners that provide the best service to your destination. We do not use any charter or unlicensed carriers. No flights will be booked without your written approval. If you have a preferred airline, please advise your Travel Specialist when scheduling your trip.
We often accommodate special requests outside of the core trip you have won, but this all depends on your specific destinations, trip details, dates of travel, etc. Your Travel Specialist will work with you during your booking process to arrange all available changes you request and advise you of any additional fees related to additional destinations or extensions.
Your Award Confirmation is valid for 3 years from the date of the event where it was awarded. Please be sure to send it to us at least 120 days before your desired departure date so we can maximize the availabilities for your trip. We maintain great vendor relations with a number of wonderful properties, resorts, hotels and other facilities at each destination worldwide. Due to availability, we won't know which property will be used until the booking process begins. All properties meet our high standards for quality and service and are highly rated by Trip Advisor. We send a large number of travelers to each property and we invite them to submit surveys regarding their trip so we may maintain our high standards of quality. Your Travel Specialist will also review the exact property with you before finalizing your trip.
That depends on your destination. For the highest quality experience, we strongly suggest you avoid the overcrowded seasons. Your Travel Specialist will cover that with you when discussing your requested departure dates. There are no fixed blackout dates for your trip. If there are seats on the aircraft and rooms on the property available from our vendors, we will book you for your requested dates.
With rare exceptions, no vaccinations or inoculations are required, even for international travel. Check with your physician or your destination countries’ website(s).
We strongly recommend you obtain travel insurance to protect you against loss or change in travel plans.
